Case management can be defined as the process of assessing a client’s needs and then administering, monitoring and supervising services to ensure that their needs are fulfilled. The main objective is to ensure that the client’s problems are addressed by using a set of supervisory strategies. Termination of services marks the final step of the relationship between a case manager and their client. Case management is an outcome driven service that takes a specified duration of time depending on the needs of the client. A case manager’s expectation is that a client will be fully responsible for adhering to the independent care. A case manager should ensure that the client achieves the goals established by the case closure guidelines. By the time the treatment is terminated, the client should be intellectually and emotionally independent and be in a position to use the available community resources to take care of themselves. The client should be informed about the length of the treatment plan in advance. In order to ensure that the termination process is smooth, the case manager should maintain a good communication with the client. Strategies for termination of a case
At the beginning of the program, a case manager should discuss the termination of services so that the client understands what is expected. The ideal condition for termination of services happens when the client is confident that he/she has achieved their desired outcomes and the case manager also agrees that the case management services are no longer needed. The client should have information about the treatment plan, the length of the treatment and the goals that should be met. Communication between a case manager and the client will keep them both informed of the program’s progress. This way, it is easier to determine whether the services should be terminated or whether the treatment plan should go on. Close monitoring helps a case manager know whether the treatment plan is effective or not. In addition, reassessments should be done regularly to establish whether there are changes that need to be introduced into the treatment plan (Ncbi.nlm.nih.gov, 1998).
A case manager helps clients to recover and regain sobriety so that their can manage their lives independently. The case manager should reduce the level of contact with the client gradually so that they can learn to function independently. In case the client is unable to complete the treatment program effectively, the case manager identifies alternative methods of achieving the desired results. For clients receiving treatment programs from within the criminal justice system, the case manager can link them up with community based services so that they can continue with their treatment programs after their release. The termination of treatment plan is a positive thing for the client because it means that they are healthy and graduated to the society equipped with the proper tools to face life.
Independent care and continued client growth
Independent care helps a client grow and become more stable because it is an individualized treatment plan that is specific to the needs of the client. Clients in a correction facility have different psychological, physical and emotional issues. There are offenders who have drug and substance abuse issues, mental illnesses, and anger issues among others. The responsibility of a case manager is to advocate for the clients and link them up with resources that can meet their needs. An individualized plan contains the suitable services that each client should receive depending on their needs. The continuity of treatment programs during transition is very essential. This is because it ensures that the client does not relapse and their reintegration into the society is without issues.
Clients should be aware of their issues and be ready to work on themselves. In order to be independent and productive citizens, all their issues should be identified and resolved. Thus, a proper independent care program requires the willingness and effort of the client. It is very essential for a case manager to develop an individualized transition plan that includes client growth and relapse-prevention strategies. In addition to this, there should be a reintegration support system that encourages the client to independently follow what is expected of them. In correction facilities, offenders who are receiving treatment rarely act on their own will. Rather, part of their rehabilitation process involves a comprehensive treatment program in order to wholly address their issues and help them become better citizens. The treatment programs in the correction facilities prepare them to a post treatment period where they act independently (Woodside, & McClam, 1998).
The most critical part of case management is the transition period. It brings to light the effectiveness of the treatment programs. If the client has successfully completed their treatment and shows signs of growth, they can be reintegrated back to the society. On the other hand, those who are yet to meet their treatment goals are monitored through community organizations. By the end of the treatment programs clients should be able to be independent and have a good judgment. This ensures that they can relate with other people well without being monitored. When clients can coexist with other people and be able to withstand the challenges that the world presents each day, the goal is achieved. This ensures that chances of recidivism are reduced.
